Anaphylaxis An EPIPEN must be provided for anaphylactic students and must be renewed prior to the EpiPen expiring date. The attached Individual Anaphylaxis Plan must be completed by a parent/guardian. The Anaphylaxis Action Plan must be completed by a doctor.

Allergic Reactions An allergy is a reaction to a specific scenario or object. Students with allergies require the attached Allergy Plan to be completed by a doctor. Your child’s medication must be provided with the Allergy plan.

Student Hay fever or Allergies Form

Asthma We require all students who suffer from asthma to provide the attached Asthma Action Plan, which is completed by a doctor. A spacer with the prescribed reliever medication must be provided in conjunction with the Asthma Action Plan.

Asthma Action Plan All students with Asthma must complete a Schools Camp and excursion form, within 2 weeks of the event. This form needs to be completed with every camp and excursion.
